Information for Referrers
We value our relationships with local GPs, paediatricians, psychiatrists and other health colleagues - to ensure referal to our practice is as simple as possible, we've outlined everything required below. We provide the following type of appointments:

Medicare Better Access Mental Health Care Plan

National Disability Insurance Scheme (NDIS -Self Managed or Plan Managed)

Private (no GP referral needed)

a request for service under the appropriate referral i.e., Better Access in the form of a referral letter

the date of referral
-
the patient’s name, date of birth and address

the referring practitioner’s name and Medicare Provider Number
-
the practitioner’s signature.

a diagnosis of a mental health disorder
-
a specified number of sessions before a review is required (e.g. 6 sessions)
